AND NOW, this 2nd day of September, 2010, COMES Pe titioner, ROBERT
ALFRED LONG, by and through his attorney, SANFORD A. KREVSKY, who petitions this
Honorable Court to appeal the suspension of his license and, in support thereof, offers the
following:
1. Petitioner, ROBERT ALFRED LONG, is a Pennsylvania resident residing at
819 Country Club Road, Camp Hill, Pennsylvania 17011.
2. On August 11, 2010, Petitioner was advised by the Department of Transportation
that his License will be suspended as authorized in Section 3362 of the
Pennsylvania Vehicle Code (Exhibit A).
3. Petitioner is permitted to appeal the suspension of his license.
WHEREFORE, Petitioner requests your Honorable Court to appeal the suspension of
his license and to stay the license suspension pending a hearing on this matter.

product(s), the DL16LC Form, or a letter acknowledging your
sanction is received in this Bureau.
WHEN THE DEPARTMENT RECEIVES YOUR LICENSE OR ACKNOWLEDGE-
MENT, WE WILL SEND YOU A RECEIPT. IF YOU DO NOT RECEIVE THIS
RECEIPT WITHIN 15 DAYS CONTACT THE DEPARTMENT IMMEDIATELY.
OTHERWISE, YOU WILL NOT BE GIVEN CREDIT TOWARD SERVING THIS
SANCTION.
