Birth Name: The minor was named Lila Grace Hughes at birth, and as of the date of this petition, the minor has always been legally known by that name. 3. Residence: The Petitioners reside at 325 16th Street, New Cumberland, PA and have lived at this address for one year. The minor child has resided at the following during the life of the child: a. 325 16th Street, New Cumberland PA, 17070 for the year preceding this petition. b. 629 Canvasbach Drive, Etters, PA 17319 for the remainder of her life. 4. Minor's Parents: The father of the minor is named Adam Paul Castner, and he resides at 325 16th Street, New Cumberland, PA 17070. The mother of the minor child is named Joanne Amber Fegan, and she resides at 325 16th Street, New Cumberland, PA 17070. 5. Reason for Name Change: Petitioners seek to change said minor's name to Lila Grace Castner and offer the following reason for seeking to do so: The minor child was born to Joanne Amber Fegan (petitioner) out of wedlock. Aaron D. Hughes was listed on the birth certificate as the father of the minor child. A paternity test was completed January 2008 verifying the paternity of the minor child for Adam Paul Castner. A copy of the paternity test is attached to this petition. Minor child resides with Petitioners and they particpate jointly in her care and upbringing. Petitioners have represented to the minor child that her father is Adam Paul Castner for all of her recollective memory. All parties to this matter agree, with the inclusion of Aaron D. Hughes on the 9a zoo at4-x name change of the minor. 6. Legal and Creditor Status: The minor does not have any outstanding judgements, has never been convicted of a crime, and is not involved in any pending legal actions. 7. Additional Declarations: In addition to the foregoing statements, the Petitioners further declare they are engaged to be married Saturday, October 23, 2010 at which time Joanne Fegan will become Joanne A. Castner.
