PETITION FOR CHANGE OF NAME
TO THE HONORABLE, THE PRESIDING JUDGE OF SAID COURT:
The Petition of Tammy Sharpe and John W. Sharpe, II, filed pursuant to 54 Pa.
Cons. Stat. Section 701 et seq., respectfully sets forth that:
1. Your Petitioners, Tammy Sharpe and John W. Sharpe, II, (hereinafter referred to
as "Petitioners" from time to time), husband and wife, are sui juris Caucasian adults.
Petitioner Tammy Sharpe resides at 301 Hollar Avenue, Shippensburg (Cumberland
County), Pennsylvania 17257. Petitioner John W. Sharpe, II, resides at 103 North
Penn Street, Shippensburg (Cumberland County), Pennsylvania 17257.
2. Petitioners were married on February 14, 1998, in Shippensburg, Pennsylvania.
3. Petitioner Tammy Sharpe is 32 years-old, having been born February 15, 1969.
She is the birth mother of Brian M. Ferry (hereinafter referred to as "Brian"), a
Caucasian male, ten (10) years old, who was born on December 1, 1990, in Carlisle
Hospital, Carlisle, Pennsylvania.
4. Petitioner John W. Sharpe, II, is 34 years old, having been born February 16,
1967. He is the step-father of Brian.
5. Petitioners resided together with Brian from May, 1997, until about January 28,
2001.
6. Petitioners have one daughter, named Cameron Rhea Sharpe, born to them on
April 24, 1999.
7. The birth father of Brian is John M. Ferry, Jr., whose parental rights were
involuntarily terminated by way of Adjudication and Decree Nisi entered in the Court of
Common Pleas in Franklin County, Pennsylvania, on September 6, 2000. The birth
father did not file exceptions and so the Decree became final on September 18, 2000.

8. Petitioner Tammy Sharpe was never married to Brian's birth father.
9. The Petitioners had intended for Petitioner John W. Sharpe, II, to adopt Brian,
after the rights of the birth father were terminated. However, the Petitioners have since
separated.
10. Brian is a resident of Shippensburg, Cumberland County, Pennsylvania, and he
has resided at the following locations during the five years next preceding the filing of
this Petition:
From June 1, 2000. to present:
301 Hollar Avenue, Shippensburg (Cumberland County), Pennsylvania 17257
From Mav, 1997, to Mav 31, 2000:
3462 Wayne Road, Chambersburg, PA 17201
From February, 1994, to May, 1997:
5075 Lincoln Way East, Suite ^, Fayeffeville, PA 17222
11. Petitioners and Brian desire that Brian's surname be the same as his birth
mother and sister.
12. Brian is not married, nor does he have any children.
13. There are no judgments or decrees of like character against Brian. ^ copy of a
lien report prepared by Capital Abstract Corporation, dated March 13, 2001, and
showing no liens against Brian, is attached hereto as Exhibit B.
14. Petitioners desire to change Brian's name to Brian Matthew Sharpe.
WHEREFORE, your Petitioner respectfully requests this Honorable Court to enter a
decree changing Brian's name from Brian Matthew Ferry to Brian Matthew Sharpe.

AND NOW, September ~ . 2000, the above matter having been heard on June 20, 2000
and consideration of all the evidence presented, as well as the arguments and briefs of the parties,
and the law, it is hereby ORDERED and DECREED that:
The rights and duties of Respondent, J.M.F., natural father of B.M.F., bom December
1, 1990, are hereby terminated.
Custody of B.M.F. is ordered to the Petitioner, T.S., his natural mother, and her
husband, Co-Petitioner, J.W.S., who shall be permitted to proceed with an adoption
action without further notification to Respondent, J.M.F.
Respondent, J.M.F., shall have ten (I0) days from the date of this Decree Nisi to file
exceptions thereto pursuant to Pa.R.C.P. 227.1. Said Decree shall become final
absent the timely filing of exceptions thereto.
A copy of this Decree Nisi and Adjudication along with the Notice of Decree Nisi
shall promptly be mailed by the Clerk of the Orphan's Court to Respondent, J.M.F.

Respondent, J.M.F., is hereby advised pursuant to 23 Pa.C.S.A. Section 251 l(c) of his
continuing right to place and update personal and medical history information on file with
the Court and with the Department of Health pursuant to 23 Pa.C.S.A. Section 2905 (d)
regarding his granting permission of the Department to disclose information contained in
B.M.F.'s original Certificate of Birth or any other identifying or non-identifying information
pertaining to the child at any time after the child attains the age of 18 or, if under the age of
18, to his adoptive parents or legal guardians.
