AND NOW, this ~~ay of September, 2010, upon the petition of Jessica E. Bergey, it
is ORDERED AND DECREED that a Citation shall be awazded, directed to Steven Joshua
Bergey to appeaz and show cause as to why he should not be declared an incapacitated person,
and Jessica E. Bergey should not be appointed as the plenary Guazdian of his person and estate.
The alleged incapacitated person shall be given at least twenty (20) days notice of the
hearing on appointment of a plenary Guazdian of his person and estate by serving him personally
with the Citation and this Order and a copy of the foregoing Petition prior to the time of such
hearing.
The time and place of hearing on the Petition for Adjudication of Incapacity and
Appointment of Plenary Guardian of the Estate and Person of the alleged incapacitated person

At least twenty (20) days written notice of the hearing on appointment of a plenary

manner: Steven G. Bergey and Swynnton J. Bergey, Stephanie J. Bergey, and Jeremiah I.
Bergey c/o Jessica E. Bergey by first class United States Mail.
