The undersigned, CATHARINE B. PETKO, being the surviving spouse
of DAVID W. PETKO, intending to be legally bound, makes the following
disclaimer:
Disclaimant hereby disclaims the right to receive the residue of the
estate of DAVID W. PETKO outright, free and clear of trust, as provided in ITEM V
of his Last Will and Testament dated August 6, 2009.
This Disclaimer is irrevocable and unqualified. Disclaimant
understands that the property will pass, pursuant to the provisions of ITEM V of
Decedent's Will, to the Petko Disclaimer Trust.
