Petition and evidence received at a hearing held thereon, this Court findfi, IIbY clear and
convincing evidence, that DUSTIN S. HOCKENSMITH is adjudged a totahyl, incapacitated
Person•
The Court finds that DUSTIN S. HOCKENSMITH is in an unresponsi~te ~dndition as a
result of a motor vehicle accident, a condition that totally impairs his capacity to i for himself,
to receive and evaluate information effectively and to make and communicate decisions
concerning his management of financial affairs or to meet essential requirements for !,his physical
health and safety. '~
Petitioner, PATTY J. DARON, his mother, is hereby appointed ple permanent
guardian of the PERSON of DUSTIN S. HOCKENSMITH. Petitioner shall in tea rapacity file
a report on the social, medical and other relevant conditions as required b~ Q Pa.C.S.A.
§5521(c).
Petitioner is also hereby appointed plenary permanent guardian of tt~e 1~STATE of
DUSTIN S. HOCKENSMITH. Petitioner shall not be required to post a Court apoved bond,
but Petitioner shall file a report in compliance with 20 Pa.C.S.A. § 5521(c). I~~
The guardian of the PERSON shall have the authority and responsibility ~o ~le~cide where
DUSTIN S. HOCKENSMTTH shall live and how meals, personal care, tra.~spo#~tation and

recreation will be provided. The guardian of the person shall also have the authority to authorize
and consent to medical treatment and surgical procedures necessary for thei, well being of
DUSTIN S. HOCKENSMITH.
The guardian of the ESTATE shall have the authori an ~I
ty d responsibility'Ito manage and
use DUSTIN S. HOCKENSMITH'S property and financial resources for his. benefit. The
guardian of the estate shall have authority over all issues relating to his cash, decks, and any
bank or savings accounts held in his name, his personal property, his entit~eiment to any
governmental and nongovernmental benefit plans, federal, state, and local taxes, c~l~ims made or
to be made on behalf of him or against him, the execution of any documents necessary and
related to the above, and the entry into contracts affecting him as well as '' the payment of
reasonable compensation or costs to provide services to him.
The aforementioned judicial determinations have taken into considera#io~ the matters
required by 20 Pa.C.S.A.§5512.1. The Court's findings of fact and conclusions of l~w have been
placed on the record at the evidentiary hearing ',
