PETITION FOR CHANGE OF NAME
NOW COMES Your Petitioner, CYNTHIA CAROL MOYER, Natural Mother
of AUSTIN ANTHONY LUNSFORD, by her attorney, Charles E. Petrie, and
respectfully represents as follows:
1. That Your Petitioner is CYNTHIA CAROL MOYER, Natural Mother of
AUSTIN ANTHONY LUNSFORD, a minor child born on March 15, 1998, and
currently residing at 227 ½ A Pennsylvania Avenue, West Fairview, County of
Cumberland, Pennsylvania, since September, 2002.
2. That Your Petitioner desires to change said minor child's name to
AUSTIN ANTHONY MOYER so that the child's last name will be the same as
Your Petitioner's.
3. That Your Petitioner believes that the said minor child's natural
father's name is MICHAEL PAULUS. The natural father's last known address is
77 North Main Street, Marysville, Pennsylvania. Said minor child last saw his
natural father in 1999.
4. That Your Petitioner has resided at the following addresses during the
past five years: From March 15, 1998, until June 2001, at 4875 Deimler Lane,
Enola, Pennsylvania, from June, 2001, until September, 2002, at 77 Autumn
Lane, Enola, Pennsylvania, from September, 2002, until the present at 227 ¥2
Pennsylvania Avenue, West Fairview, Pennsylvania.
WHEREFORE, Your Petitioner respectfully requests that Your Honorable
Court pursuant to 54 Pa.C.S. § 701, to:
(a) fix a hearing date for this Petition; and
(b) direct that notice be given of the filing of this Petition and of the
hearing.

AFFIDAVIT OF INVESTIGATION
CHARLES E. PETRIE, duly sworn, hereby deposes and says:
1. That he is the Attorney for the Petitioner in the above-captioned
matter.
2. That the last known residence of the natural father, MICHAEL
PAULUS, in the above-captioned matter was 77 North Main Street, Marysville,
Pennsylvania.
3. That natural father last saw said minor child in 1999.
5. That the following steps have been taken in an effort to serve said
natural father with notice of the Change of Name Hearing:
a. A Notice was sent to Mr. Paulus at his Marysville address on
July 22, 2004, by certified mail, restricted delivery. Said envelope was
returned marked ~'Refused." A copy of said envelope i:s attached hereto.
b. Three attempts to serve Mr. Paulus at his residence by Process
Server were made on the following dates and times:
1. Wednesday, August 4, 2004, at 7:00 P.M., no answer at
the residence.
2. Monday, August 9, 2004, at 8:00 P.M., no answer at the
residence.
3. Saturday, August 14, 2004, at 10:00 A.M. A woman
answered the door and stated that Mr. Paulus left there four weeks ago and
has not returned. The process server saw a male individual looking out the
second story window as he drove away. The woman who answered the door
later left a message on the answering machine of Mr. Petrie indicating that if
the process server returned to their residence she would have him charged with
harassment.
c. On August 16, 2004, a copy of the Order and Notice of Change
of Name Hearing was sent to Mr. Paulus at his Mary,,~ville address by First
Class Mail, Postage Prepaid, with a Certificate of Mai]ling. A copy of the
Certificate of Mailing is attached hereto. This envelope has not been returned.
6. The Sentinel published Notice of the Change of Name hearing on July
24, 2004, and the Cumberland Law Journal published Notice on July 30,
2004. Copies of the Proofs of Publication are attached hereto.
7. On August 17, 2004, an attempt to serve Mr. Paulus at his last
known employer, Hempt Bros., was made. The process server was informed
that Mr. Paulus' employment was terminated as of April 9, 2004.
8. That Petitioner has no information re§arding the location of any of Mr.
Paulus' relatives.
9. That Petitioner has no other information concerning the whereabouts
of the natural father.
