TO THE DEFENDANT: The above named plaintiff(s) asks judgment against you for $ 2229.34 together with costs
upon the following claim (Civil fines must include citation of the statute or ordinance violated):
Plaintiff acquired the obligation originally issued by Chase Bank. Plaintiff demands of Defendant(s)
the unpaid principal balance due under the credit card agreement Visa Acct. # ?.. There is due and
owing from defendant the sum of $1879.34, and a reasonable attorney's fee of 350.00, or a total of $2229.34. See
attached statement and Verification, attached (Exhibit "A").
In accordance with the Fair Debt Collection Practices Act, 15 U.S.C.A. 1692(g)(a) on January 26, 2010, Plaintiffs
counsel sent a written notice to Defendant. To date, no response has been received. (See Exhibit "B").
