due notice according to law and upon consideration of the within Petition and the Motion
of Wayne F. Shade, Esquire, attorney for Ann P. Barzda, the Court finds that the welfare
of Mary A. Barzda would not be promoted by her presence in Court.
The Court further finds, as follows:
1. By reason of a clinical diagnosis of senile dementia of the Alzheimer's type,
Mary A. Barzda is impaired in the capacity to make and communicate decisions for the
management of her financial resources or to meet the essential requirements for her
physical health and safety.
2. The extent of the capacity of Mary A. Barzda to make and communicate
decisions is total.
3. The nature of the condition or disability of Mary A. Barzda is such as to require
the appointment of a plenary Guardian of the estate and of the person of Mary A. Barzda.
4. The duration of the guardianship shall continue in effect until further Order of
WAYNE F. SHADE ul1S COUrt.

5. Ann P. Barzda of 103 Broad Street, Mount Holly, New Jersey 08060, is
appointed plenary Guardian of the estate of Mary A. Barzda with authority to make
distributions from the principal of the assets of Mary A. Barzda for the care of Mary A.
Barzda as necessary.
6. Ann P. Barzda is appointed plenary Guardian of the person of Mary A. Barzda.
7. The said Guardian of the estate is directed to file an inventory in accordance
with the provisions of § 5142 of the Probate, Estates and Fiduciaries Code within three
months of the date of this Order.
8. The said Guardians are excused from filing bond.
