Petitioner previously resided at 509-B Crowne Plaza Apartments, Mckeesport, Pennsylvania, 17132. 4. Petitioner was born on 12/30/43 in Webb County, Texas. Petitioner's name on her birth certificate is Esther Elisa Saenz. 5. Petitioner was married on 7/13/1964 to Peter S.C. Petitioner was married on 4/11/1973 to James Richard Pike and took her husband's last name, thereby becoming Ether Elisa Saenz Molnar Pike. 7. Petitioner has been using her given middle name, Elisa as her first name, rather than her given first name of Esther, since she was a child and wishes to legally change her name to reflect this. 8. Petitioner did not divest herself of her ex-husbands' family name, Molnar, at the time of her divorce in the early 1970's and wishes to now do so. 9. Petitioner has not used Saenz since her first marriage and wishes to officially divest herself of it at this time. 10. Petitioner would therefore legally be known as Elisa Pike, born Saenz.
