AND NOW, this o?~ day of October, 2010, upon consideration of the attached Petition of
Justin Michael Holliday for the change of name, it is hereby Ordered and directed as follows:
1. A hearing is scheduled in Court Room Number 5 of the Cumberland County
Courthouse on the ~,~ day of ~a'r,~,,,~i~ , 20 ~ o , at ~: ~fS 4 .m.
2 Petitioner is directed to give notice of the filing of this Petition and of the date of the
hearing by publication consistent with the requirements of 54 PA. C.S.A. Section
701(A.1)(3)(ii)(A).
3. Petitioner is directed to comply with the requirements on 54 PA. C.S.A. Section 702
by furnishing a duplicate copy of this Petition and a set of the Petitioner's fingerprints
to the Cumberland County Prothonotary for forwarding to the Pennsylvania State
Police.

AND NOW, this day of November, 2010, upon consideration of the Petition of Justin

Michael Holliday for the change of name of Petitioner, and after a hearing on said Petition, and it
appearing that the Petitioner has complied with the requirements of 54 PA. C.S.A. Section 702
relative to a criminal record check and the requirements of 54 PA. C.S.A. Section 701 with respect
to advertisement of the hearing, and it further appearing that Petitioner has met all the other
requirements of 54 PA. C.S.A. Section 701 et seq. with respect to a name change and there being no
lawful objection to the request of Petitioner, it is hereby ordered that the name change Petition is
granted and Justin Michael Holliday's name is hereby changed to Justin Michael Moor.
