The matters and things in controversy having been discussed by and between the partie
and a settlement having been agreed upon:

ASSOCIATES, LLC; ("Plaintiff?") laid 13ARBARA A HOOVER ("Defendant"), as follows:
1. Plaintiff tiled suit in the above captioned matter seeking damages in the arnount of
$2,028.39, plus court costs in the amount t,f $130.$0, for a total of $2,159.19.
2. Defendant agrees to pay-to Plaintiffthe sum of $1,704.00, which Plaintiff agrees to ace Pt
in full settlement of its claim herein.
3. As of this date, payments totaling $0.00 have been applied to the aforementioned sum.
4. Wfendant shall remit payment(s) in the following manner:
a. $'284.00 to be paid on or before the last day ol'each month, beginning May 31,
2012 through October 31, 2012.
