MUNICIPAL LIEN FOR SEWER ASSESSMENT AND SEWER FEES
Carlisle Suburban Authority, North Middleton Township, County of Cumberland,
Commonwealth of Pennsylvania, by its solicitor, Broujos & Gilroy, P.C., hereby files its
claim for sewer assessment against the real estate hereinafter described and sets forth its
claim as follows:
STATEMENT OF CLAIM
1
The name of the municipality by which this claim is filed is Carlisle Suburban Authority,
a municipal authority of the Commonwealth of Pennsylvania, located in the County of
Cumberland and organized pursuant to ordinances of North Middleton Township.

The name of the owners of the property against which this claim is filed are Robert H.
Culley and Elizabeth A. Culley, with the address of the subject property at the following:
375 Longs Gap Road
Carlisle, PA 17013

A description of the property against which this lien is f'fled is attached hereto and
marked Exhibit "A".

The authorities under and by virtue of which this lien is filed is the Municipal Claims and
Liens Act, 53 P.S. §7101 et seq.

This lien is claimed for assessment issued against the above described property for
installation of a sewer connection in the amount of $4,600.00 plus accrued sewage user
fees through December 21, 2001 in the amount of $813.57 for a total of $5,413.57.

Interest is claimed on the above-mentioned amount from December 21, 2001 and costs of
filing are also claimed.
