( ) A criminal record was revealed containing felony convictions. IN ACCORDANCE WITH THE NAME
CHANGE ACT, THE COURT SHALL NOTIFY THE PENNSYLVANIA STATE POLICE WHEN A NAME
CHANGE FOR A. PERSON CONVICTED OF A FELONY HAS BEEN ORDERED. The Pennsylvania State
Police, upon receipt of this notice, shall include the change of name information in the Central Repository as
provided for in 18 PA CS., Chapter 91 (relating to criminal history record information).

Pro Se Petition for Change of Name,
IT IS HEREBY ORDERED AND DIRECTED that a hearing will be held on Tuesday,
March 1, 2011, at 2:00 p.m. in Courtroom No. 2 of the Cumberland County Courthouse,
Carlisle, Pennsylvania.

AND NOW, this 1st day of March 2011, after a
hearing in the above-captioned matter, the court does find that
the petitioner has filed the required documents, has provided the
court with an adequate reason for the name change, and there
being no legal or other objection to the name change; IT IS
HEREBY ORDERED AND DIRECTED that Anthony Holloway's current name
is hereby changed to Scarlett Iris Holloway.
