AND NOW, on this / ~" day of ~yOV~f./l. , 2010, in
consideration of the aforegoing Petition to Appoint Guardians of the Property of a Minor Child
and subsequent Stipulation, it has been determined that Jessica Nicole Guest is in need of a
guardian to be appointed for her property.
NOW, THEREFORE, based on the clear and convincing evidence supporting the
foregoing fmdings IT IS ORDERED, ADJUDGED AND DECREED that April L. Casper and
Mark A. Casper shall be appointed as Guardians of the property of the minor child, Jessica
Nicole Guest.
Guardians shall be empowered to accept the life insurance proceeds of which the
minor child is a beneficiary in order to deposit them and safeguard them as more fully set forth
as follows:
a. Guardians shall place the life insurance proceeds in a blocked account
in a federally insured savings institution with the highest interest rate available in
the Central Pennsylvania area. Guardians. would reserve the right to request the
Court for permission to access these funds to meet the necessary expenses of the
child, should the need arise; and

b. Five Thousand Five Hundred Dollars ($5,500) shall be set aside from
the aforementioned proceeds so that the minor child, Jessica Nicole Guest may
use it towazds her color guard expenses. It is anticipated that the child will need
approximately One Thousand Two Hundred Dollars to One Thousand Five
Hundred Dollars ($1,200 to $1,500) for the winter season and Four Thousand
Dollars ($4,000) for the summer program.
Guardians shall not be required to post bond.
Jessica Nicole Guest or her agent have 20 days from the date of this decree to file
exceptions. Failure to file exceptions within that time will result in this decree become final.
Jessica Nicole Guest or her agent have been advised of the right to appeal and to
petition to modify or terminate the guardianship by copy of this decree and by the Statement of
Rights attached hereto.
