North Middleton Authority, North Middleton Township, County of Cumberland, Commonwealth of
Pennsylvania by its solicitor, Martson Deardorff Williams Otto Gilroy & Faller, hereby files its
claim for sewer services against the real estate hereinafter described and sets forth its claim as
follows:
STATEMENT OF CLAIM
The name of the municipality by which this claim is filed is North Middleton Authority, a municipal
authority of the Commonwealth of Pennsylvania, located in the County of Cumberland and
organized pursuant to ordinances of North Middleton Township.

The name of the owner of the property against which this lien is filed is Edward A. Lepard, 1180
Newville Road, Carlisle, North Middleton Township, Pennsylvania 17013.

A description of the property against which this Lien is filed is attached hereto and marked as
Exhibit "A" with an address of 1180 Newville Road, Carlisle, Pennsylvania 17013.

The authorities under and by virtue of which this Lien is filed is the Municipal Claims and Liens Act
of May 16, 1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq. dD4A{J~9"

The lien is claimed for installation of a sewer lateral consistent with a written agreement between
the parties and is claimed in the amount of $4,107.59, plus fees of recording and interest from
November 1, 2010.

Interest is claimed on the above-mentioned fees until paid in full.

County, Pennsylvania,

WITNESSETH, that inconsideration of One Hundred Twelve Thousand Dollars
(S112,000.00), in hand paid, the receipt whereof is hereby acknowledged, the said Grantor do

ALL THOSE CERTAIN two certain tracts of land with the improvements thereon emted
situated in North Middleton Township, Cumberland County, Pennsylvania bounded and
described as follows:
TBASZ1.: BEGINNING at a point in the South Side of the Newville-Carlisle Road,
Pennsylvania Highway Route N641 at wrner of Isnds of Francisus; thence in a Southerly
direction along the Francisus land 182 feet to the Northern side of an alley; thence by the
Northern side of said alley in a Westerly direction 75 feet to an iron pin;: thence by other land
retained by Grantors in a Northerly direction 182 feet to the South Side of said Road; thence
the South Side of said road in an Easterly direction TS feet to the place of BEGINNING.
BEING the sattte premises which Richard W. Collins and Mildred P. Collins, his wife, by
their dated May 31, 1962 and raorded in the Office of the Recorder of Deeds in and for
Cumberlatd County in Deed Book 0, Volume 20, Page 981, granted and conveyed unto
Anthony Gromack and Marie E. Gromack, his wife. Anthony Gromack died January 6, 1986
thus vesting full fee simple title in Marie E. Gromack, grantor herein.
TBA~Z2: BEGINNING at a point on the South side of the Newville-Carlisle Road,
Penna. Highway Route No, 641, at the Northwest corner of land heretofore conveyed to titc
within Grantees, which point of beginning is 75 feet West of land of Franciscus; thence in a
Southerly direction along said other land of the within Grantees, a distance of 182 feet to -he
Northern side of an alley; thence by the Northern side of said alley in a Westerly direction, a

distance of 10 feet to a point; thence by other land retained by the Grantors in a Northerly
direction, a distance of 182 feet to the South side of said Newville-Carlisle Road; thence by
the South side of said road in an Easterly direction, a distance of 10 feet to a point, the Place
of BEGINNING.
CONTAINING 10 feet in front along the Southern line of the Newville-Carlisle Road and
extending Southwardly therefrom at an even width a distance of 182 fat to the North side of
an alley.
BEING the same premises which Richard W Collins attd Mildred P. Collins, husband and
wife, by their deed dated November 26, 1963, and recorded In the Office of the Recorder of
Deeds in acrd for Cumberland Courky in Deed Book B, Volume 21, Page 199, granted and
conveyed uMO Anthorty Gromack and Marie E. Gromack, husband and wife. Anthony
Gromack died January 6, 1986 thus vesting full fee simple title in Marie E. Grotnack, grantor
herein.
And the said Grantor hereby covenant and agree that they will warrant specially the property
hereby conveyed.
