November 15, 2010 ~~ `-~'' ~~ ~ ~' '' Report for Guardian of Person ~ ~ r Cumberland County Aging and Community Services ~ rv ,. ~-, Priscilla Whitman, Aging Care Manager 3 «> On July 31, 2008 an Emergency Guardian hearing was held before Judge Hess. He appointed the Cumberland County Office of Aging as Emergency Guardian. Mr. Gallagher was taken to the Eall CTns ans and x-rays were taken and he wlas found to and leg pain as a result of a have a slight hip fracture but nothing that required surgery. He was then admitted to Forest Park Nursing Center where his stay is expected to be long term. New clothes were purchased for him. He adjusted well and seemed pleased to receive care. He received a bath and a hair cut and later was seen by a dentist. A few days after Bill's admission to the nursing home his son, Jimmy, passed away. Aging staff broke the news to Bill. They later took him to view his son at the funeral home. On August 27, 2009 the final guardianship hearing took place before Judge Hess. The agency was appointed Permanent Plenary Guardian of Person and Estate. Initially Bill was in the regular nursing unit at Forest Park but his behavior became such hat he kept going into other residents' rooms. He would disarm the alarms on the chairs of other clients which would be placing them at risk. He would randomly target people. After a trial visit which went well he was moved to the dementia unit. He continues to be delusional, claiming to have married and had two sons. He showed the case worker a picture of a car in a magazine saying he had purchased it for his wife. At one point he believed he was married to a staff member on the 3 -11 shift. Once when she was unable to come to work because of a snow storm he became angry and tried to choke another nurse. They psychiatrist who sees Bill was notified and placed him on Seroquel 25mg daily. Because of the he dement a unittS nce'the wasf moved, heshas no targeted he should be moved out of t anyone else. He continues to require extensive hands on assistance with all his activities of daily living including dressing, grooming and personal care. He is incontinent of bladder. He F:MAII. He spends his day in a wheelchair which he is able to self propel His diagnoses include dementia, anxiety, osteoporosis, mild degenerative joint disease, depression, gastroenterology reflux disorder. His medications include caldarb 600 mg, citalopram 40mg, ferrous sulfate F/C 325 (65mg), omeprazole 20mg, vitamin B-12, vitamin D3, Seroquil 50 mg TID and Ativan TID. On an as needed basis his medications include acetaminophen 325mg, apap supp 650mg, milk of magnesia 400mg/5m1. It is the request of the Cumberland County Office of Aging that the agency retain Guardianship of Person for William Gallagher. Forest Park Nursing Center has demonstrated an adequate quality of care for her medical and mental .problems. We believe, therefore, that William Gallagher should continue to reside in that nursing facility.
