Buckley, Esquire, and request that this Court order a change of name for their minor child, Allen Tseng, pursuant to 54 Pa. Cons. Stat. Ann. § 701 et seq., and Local Rules of Court, Rule 5 and in support thereof represent as follows: 1. Petitioners, Mu Ching Chu, the biological father of minor child Allen Tseng, and Hsiu Lin Tseng, the biological mother of minor child Allen Tseng, are both residents of 318 Acre Drive, Carlisle, Cumberland County, Pennsylvania, and both have resided at that residence with their minor child Allen Tseng, during the five years next preceding the filing of this petition. 2. Petitioners' minor son was born in Carlisle, Cumberland County, Pennsylvania on September 16, 2005 as Allen Tseng and has since birth resided with Petitioners at 318 Acre Drive, Carlisle, Cumberland County, PA 17013. 3. The Certificate of Birth for the minor child Allen Tseng does not record any information regarding the father of the child and Petitioner Mu Ching Chu was inadvertently not listed as the father of said child. (A true and correct copy of the Certificate of Birth is attached hereto and made a part hereof and marked as Exhibit "A") VaRX 00 Qx4v q v-# a5 VT 4. Petitioners' minor son was given mother's last name on his birth certificate and this fact was not noted until recently. 5. Petitioners' minor son will be entering the public school system next year, 2011 and both parents desire that their son's last name be the same as his father's last name, that being Chu. 6. There are no judgments or decrees of like character of record against the petitioners' minor son, nor are there any debts or assets in the petitioners' minor child's name. 7. The minor child is under the age of six(6) years, therefore no finger printing should be required. Further, the Court directs that the Original Certificate of Birth for the minor child Allen Tseng shall be returned to the Commonwealth of Pennsylvania with a certified copy of this Order and a new Certificate of Birth shall be issued for the Allen Tseng Chu naming Hsiu Lin Tseng as the mother and Mu Ching Chu as the father of the child Allen Tseng Chu. l,1 AIN 411,111_ / BY THE COURT J.
