u on consideration of the Petition for Creation of a Trust filed in

the above-captioned proceeding, it is hereby ORDERED AND DECREED
that the Norma Frame Special Needs Trust be and the same is hereby
established with and subject to the terms and conditions as set
forth in the trust instrument for the same which is attached hereto
and made a part hereof; and that, before Petitioner transfers any
funds to the said Trust, Petitioner shall pay to the Department of
Public Welfare the sum of $69,152.99 which shall fully satisfy the
current Department of Public Welfare claims for assistance.
