FINAL ORDER
AND NOW, to wit this the ~~day , 2011, it is lfe~by
ordered that the settlement in the amount of Fifteen Thousand ($15,000.00) I)ol~ats is
granted. The proceeds aze to be distributed as follows:
(a) The net wrongful death proceeds of $5,625.OOwi11 be placed in a ftd~r~lly
insured bank or saving institution as permitted by the Rules of Civil ocedure
for benefit of the minor children, to be divided equally into account~~s~~I,f~r each
of the minor children. Said account to be endorsed to prohibit wit~z~~`"al of
the monies therein until the child is eighteen or further order of court, i$sues.
(b) The net survival action monies of $5,625.OOwil1 be made part of the ~~tate of
Steve Dollins and distributed in accord with the accounting thereof t~ be
produced when the estate is closed.
(c) An amount of $3,750.00 of the proceeds will be paid to Rominger and'
Associates, pursuant to the fee agreement executed by the estate.
(d) All sums set forth above constitute damages on account of personal
injury/wrongful death, arising from the occurrence, within the meaGni#~g of §
104 (a) (2) of the Internal Revenue Code of 1986, as amended.
