PURSUANT TO 54 Pa.C.S.A. SECTION 701 et seq.
AND NOW, comes Aja Marie Reed, by and through her attorney, Michael A.
Scherer, Esquire and respectfully represents as follows:
1. The Petitioner is Aja Marie Reed, born December 17, 1984, who resides
at 2148 Douglas Drive, Carlisle, Cumberland County, Pennsylvania.
2. Petitioner's natural mother is Gina M. Bell, who is an adult individual who
resides at 2148 Douglas Drive, Carlisle, Cumberland County, Pennsylvania.
3. At the time of birth, Petitioner was given the name Aja Marie Reed, which
surname is that of her then step-father, James Reed. A copy of the Petitioner's birth
certificate is attached hereto and marked as "Exhibit A."
4. In 1986, Petitioner's natural mother was divorced from James Reed, and
thereafter the natural mother married Bradley E. Bell.
5. Since natural mother's marriage to Bradley E. Bell, Petitioner has used
the name Aja Marie Bell.
6. Petitioner's social security card lists the name Aja Marie Bell, and
Petitioner is registered in school and is commonly known as ^ia Marie Bell.
7. Bradley Bell has never adopted the Petitioner, and the whereabouts of
Petitioner's natural father are unknown.
8. Petitioner has resided continuously in Cumberland County for fifteen and
one-half years and has no criminal record.
9. Them are no judgments or decrees of record against Petitioner.
10. Petitioner now desires Court approval to have her name changed to Aja
Marie Bell so that she can make application to have her birth certificate reflect the name
she is actually using.
11. Petitioner has been denied a driver's license in Pennsylvania because the
name on her birth certificate does not match the name on her social security card.
WHEREFORE, Petitioner respectfully requests that this Honorable Court enter
an order granting Petitioner's request to change her name from Aja Marie Reed to Aja
Marie Bell.
