Wallet, it is ORDERED that all proceeds from the life insurance policy issued by
Massachusetts Mhtual Life Insurance Company to Robert F. Wallet, now deceased, be
distributed to Gralce M. Wallet, wife of Decedent.
This decree of distribution shall be accepted with respect to the above-referenced life
insurance policy,,as well as any other assets later discovered to be in the name of Robert F.
Wallet or payable to Robert F. Wallet.
This decree of distribution shall constitute sufficient authority to all transfer agents,
registrars, and anp~ other entities dealing with the property of Decedent Robert F. Waltet to
recognize Grace ~vl. Wallet as entitled to receive such property without administration, and
shall in all respects have the same effect as a decree of distribution after an accounting by a
personal represerutative.
