TO THE HONORABLE, THE JUDGES OF SAID COURT:
The Petition of FREDERICK H.. FINK, by and through his attorney, Paul
W. Kilgore, respectfully represents that:
1. Your Petitioner, Frederick H.. Fink, is an adult individual who
resides at 4173 Grouse Court # 104, Mechanicsburg, Cumberland County,
Pennsylvania.
2. Your Petitioner is a U.S. C'itizen, single and was born April 18,
1933.
3. Your Petitioner has resided at the following locations for the past
five (5) years'
a. 4173 Grouse Court #104, Mechanicsburg, PA 17050
for the past three (3) years; and
b. 9 Lucky Lane, Washingtonville, N.Y. 10992 for three
· (3) years.
4. Your Petitioner desires to change his name to Peter Iscariot, which
he has used a an illustrator, lecturer and author.
5. There are no judgments or decrees of like character of record
against Petitioner.
6. There are no outstanding loans to which your Petitioner is
obligated.
7. Your Petitioner has been finger printed and a copy is attached
hereto as Exhibit "A".
WHEREFORE, Petitioner prays this Honorable Court to enter a decree
changing his name from:
