Kilgore, respectfully represents that: 1. Your Petitioner, Frederick H.. Fink, is an adult individual who resides at 4173 Grouse Court # 104, Mechanicsburg, Cumberland County, Pennsylvania. 2. Your Petitioner is a U.S. C'itizen, single and was born April 18, 1933. 3. Your Petitioner has resided at the following locations for the past five (5) years' a. 4173 Grouse Court #104, Mechanicsburg, PA 17050 for the past three (3) years; and b. 9 Lucky Lane, Washingtonville, N.Y. 10992 for three · (3) years. 4. Your Petitioner desires to change his name to Peter Iscariot, which he has used a an illustrator, lecturer and author. 5. There are no judgments or decrees of like character of record against Petitioner. 6. There are no outstanding loans to which your Petitioner is obligated. 7.
