OF NAMF, The Petitioners, Timothy J. Perry and Brenda R. Perry, through their undersigned counsel, request that this Court order a change of name for Elisa Marie Piccirilli, pursuant to 54 Pa.C.S.A. {}701 et seq., and in support thereof represent as follows: 1. Petitioners reside at 226 South High Street, Meehanicsburg, Cumberland County, Pennsylvania, and have resided at this location for more than five (5) years. 2. Petitioner Brenda R. Perry, formerly Brenda R. Pieeirilli, is the natural mother of Elisa Marie Pieeifilli; Petitioner Timothy J. Perry is the stepfather of Elisa Marie Piecirilli. 3. Elisa Marie Pieeifilli was bom on February 12, 1985 and is presently a tenth grade student at Trinity High School. A copy of her birth certificate is attached as Exhibit "A" to this Petition. 4. Elisa Marie Pieeirilli has resided exclusively with her mother, Brenda R. Perry, since her birth, and has resided with her stepfather, Timothy J. Perry, since the marriage of he and her mother on December 1, 1990. 5. Elisa Marie Pieeirilli currently resides with her mother, stepfather, and three brothers: Andrew Timothy Perry, bom March 25, 1991; Anthony Joseph Perry, bom July 14, 1992; and Austin Michael Perry, bom August 11, 1994. 6. It is the desire of the Petitioners and of Elisa Marie Piecirilli to change the name of Elisa Marie Piceirilli to Elisa Marie Perry. 7. The requested change of name is in the best interests of the minor child, Elisa Marie Piceifilli, so that she may have the same name as her mother, stepfather, and brothers, and the child does understand the significance of changing her name. 8. There are no judgments or decrees of like character of record against either the Petitioners or against Elisa Marie Piccirilli.
