Bangs, Esquire, and files the following Petition for Name Change: 1. Petitioner, JONATHAN MICHAEL DEJESUS-BANGS, is an adult individual who resides at 431 Parkside Drive, Camp Hill, Cumberland County, Pennsylvania, and who has resided at that address for a period in excess of five years prior to the time of filing of this Petition. 2. Petitioner was born in Puerto Rico and upon his birth, his last name was a combination of his father's and mother's last names, hence the hyphenated last name. 3. Petitioner has always used the name Jonathan Michael Bangs without reference to his natural father's last name, with whom he has had no contact for a period of almost twenty years. 4. There have been occasions when Petitioner's non-use of the hyphenated last name has created confusion and he wishes to eliminate that problem in the future. 5. Petitioner is not trying to avoid any outstanding debts; to avoid any creditors; or is engaging in any type of fraudulent activities in requesting this name change. 6. Petitioner is not a convicted felon nor has he had any criminal convictions and is not seeking to change his name as a result of any conviction. 7. Petitioner is requesting this name change so that he may formally and legally use the name that he has been accustomed to using, Jonathan Michael Bangs, and eliminate the need to reference the name on his birth certificate, Jonathan Michael Dejesus-Bangs. 8. There are no minor children that are affected by this name change. WHEREFORE, Petitioner requests this Honorable Court to grant Petitioner's request for a name change such that Petitioner may be permitted to use the name Jonathan Michael Bangs.
