ORDER OF COURT
AND NOW, this 16~' day of February, 2011, upon consideration of ~~ Amended
Petition To Approve Settlement of Wrongful Death and Survival Actions, it is hereby
ordered and directed that Misty Clifford, Administratrix of the Estate of 11rl~chael Alan
Clifford, II, deceased, is authorized to enter into asettlement with Natio$~al Mutual
Insurance Company, for payment of benefits in the gross amount of Clr~e Hundred
Thousand Dollars ($100,000.00), arising out of the death of Michael Alan C~i~ford, II, in
a January 19, 2010 motor vehicle accident, with one hundred percent (100%~ allocated to
the wrongful death action and zero percent (0%) allocated to the survival actib#~.
The settlement proceeds shall be allocated and distributed as follows:
Wrongful Death Action.
To: Misty Clifford (mother of decedent)
Survival Action
To: Misty Clifford (mother of decedent)
