claim for water and sewer services against the real estate hereinafter described and sets forth its
claim as follows:
STATEMENT OF CLAIM
The name of the municipality by which this claim is filed is North Middleton Authority, a municipal
authority of the Commonwealth of Pennsylvania, located in the County of Cumberland and
organized pursuant to ordinances of North Middleton Township.

The name of the owners of the property against which this lien is filed are Robert D. Galaskas and
Kathem E. Galaskas, 816 Forbes Road, Carlisle, Pennsylvania 17013.

A description of the property against which this Lien is filed is attached hereto and marked as
Exhibit "A" with an address of 1882 Spring Road, Carlisle, North Middleton Township,
Pennsylvania 17013.

The lien is claimed for unpaid water and sewer user fees against the above-described property in the
amount of $401.16, plus any additional fees incurred thereafter.

remainders, rents, issues, and profits thereof; and all the estate, right, title, interest, property, claim and
demand whatsoever of it, the said grantor, as well at law as in equity, of, in and to the same.
To have and to hold the said lot or piece of ground described above, with the buildings and
improvements thereon erected, hereditaments and premises hereby granted, or mentioned and intended so
to be, with the appurtenances, unto the said Grantees, their heirs and assigns, to and for the only proper
use and behoof of the said Grantees, their heirs and assigns, forever.
And the said Grantor, for itself and its successors, does, by these presents, covenant, grant and agree, to
and with the said Grantees, their heirs and assigns, that it, the said Grantor, and its successors and assigns,
all and singular the hereditaments and premises herein described and granted, or mentioned and intended

ALL THAT CERTAIN tract of land situate in North Middleton Township, Pennsylvania bounded and described as
follows:
BEGINNING at a point in the center of the State Highway Route No. 34 known as SterretPs Gap Road at comer of
Lot NO. 41 in the hereinafter mentioned Plan of Lots; thence by the dividing line between Lots Nos. 40 and 41 on
the said Plan West 200 feet to a point; thence by land now or formerly of Lloyd C. Brumbaugh and his wife, South
75 feet to a point; thence by the center of Lot No. 39 in the hereinafter mentioned Plan of Lots East 200 feet to a
point in the center of the aforesaid State Highway; thence by the center of the aforesaid State Highway North 75
feet to a point the Place of BEGINNING.
