North Middleton Authority, North Middleton Township, County of Cumberland, Commonwealth of
Pennsylvania by its solicitor, Martson Deardorff Williams Otto Gilroy & Faller, hereby files its
claim for sewer services against the real estate hereinafter described and sets forth its claim as
follows:
STATEMENT OF CLAIM

The name of the municipality by which this claim is filed is North Middleton Authority, a municipal
authority of the Commonwealth of Pennsylvania, located in the County of Cumberland and
organized pursuant to ordinances of North Middleton Township.

The name of the owner of the property against which this lien is filed is Shawn W. Hurley, 725
Longs Gap Road, Carlisle, North Middleton Township, Pennsylvania 17013.

A description of the property against which this Lien is filed is attached hereto and marked as
Exhibit "A" with an address of 725 Longs Gap Road, Carlisle, Pennsylvania 17013.

The authorities under and by virtue of which this Lien is filed is the Municipal Claims and Liens Act
of May 16, 1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq. `?

The lien is claimed for unpaid sewer user fees against the above-described property in the amount of
$256.24, plus any additional fees incurred thereafter.

Interest is claimed on the above-mentioned fees until paid in full.

in North Middleton Township, Cumberland County, Pennsylvania, bounded and described
according to a survey made by D. P. Raffensberger, Registered Surveyor, as follows:
BEGINNING at a point in the center of Longs Gap Road, said point being 300 feet
measured North 22 degrees 30 minutes West of the intersection of the center line of Longs
Gap Road and the center line of the Township road leading from Longs Gap Road to
Pennsylvania Route No. 34; thence by the center of Longs Gap Road, North 22 degrees 30
minutes West, a distance of 110.23 feet to a point; thence by the southern side of a proposed
street 33 feet wide, North 67 degrees 30 minutes East, a distance of 166.5 feet to a stake;
thence by lands now or formerly of Arthur R. Klinger et ux, South 22 degrees 30 minutes

East, a distance of 110.23 feet to a stake; thence by the same, South 67 degrees 30 minutes
West, a distance of 166.5 feet to a point in the center of Longs Gap Road, to the point and
place of BEGINNING.
CONTAINING 18,353.295 square feet, more or less.
BEING the same premises which Joyce L. Henry, widow, by her Deed dated
October 25, 2007 and recorded in Cumberland County as Instrument No. 200741234,
granted and conveyed unto Harry E. Line and Kathy E. Line, husband and wife, Grantors
herein.
The said grantors hereby covenant and agree that they will warrant specially the
property hereby conveyed.
