Plaintiff State Farm Mutual Automobile Insurance Company is an insurance carrier licensed to conduct business in the Commonwealth of Pennsylvania and having as one of its principal places of business the above-captioned address. 2. Theresia Moody is an adult, and on or about February 2, 2010, was the registered owner of a 2000 Chrysler Town & Country motor vehicle insured under a policy of insurance issued by State Farm Mutual Automobile Insurance Company. 3. Defendant Schmidt's Bakery d/b/a Schmidt Baking Co. ("Schmidt") is a business entity with a principal office located at 7801 Fitch Lane, Baltimore, Maryland 21236, and on or about February 2, 2010, was the registered owner of a 1990 Grumman Olson motor vehicle bearing PA Tag No.: YGP6254. 4. Defendant Victor Chiru is an adult residing at 2753 Carnegie Road, Apt. 204, York, Pennsylvania 17402 and on or about February 2, 2010, was the authorized operator of the aforesaid Grumman motor vehicle. On or about February 2, 2010, while operating the aforesaid Grumman motor vehicle during the course of his employment with Defendant Schmidt, at a PNC Bank parking lot in Camp Hill, Pennsylvania, Defendant Victor Chiru attempted a left turn onto Carlisle Road and struck Plaintiff's insured's vehicle, which was operating with the right of way resulting in extensive damage thereto. 6. The aforesaid accident was a direct and proximate result of Defendant(s) negligence, carelessness, recklessness and/or other liability-producing conduct. 7. Pursuant to the insurance policy herein mentioned, Plaintiff State Farm Mutual Automobile Insurance Company a/s/o Theresia Moody has incurred and paid expense and other damages in the amount of $5,226.74. WHEREFORE, Plaintiff State Farm Mutual Automobile Insurance Company a/s/o Theresia Moody demands judgment against the Defendant(s) in the amount of $5,226.74, which includes the Plaintiff's insured's deductible, plus interest, attorneys' fees, and costs of suit.
