payment of settlement monies was made by Nationwide Mutual Insurance Company
to Misty Clifford, mother of Michael Alan Clifford, II, deceased, pursuant to the
Order of Court dated February 16, 2011. I understand that the statements made
herein are subject to the penalties under 18 Pa.C.S. § 4904 relating to unsworn
falsification to authorities.
