PRELIMINARY DECREE
AND NOW, THIS 7~day of ~ , 2011, in consideration of the
foregoing petition and on motion of Petitioner, Presbyterian Senior Living, it is hereby
ORDERED and DECREED that a Citation be awarded, directed to Margaret H. Fick, to
show cause why a Permanent Plenary Guardian of her Person and Estate should not
be appointed, and why she should not be adjudged an incapacitated person.
The time and place of hearing on the Petition for Appointment of Permanent
Plenary Guardian of the Person and Estate of the alleged incapacitated person are
fixed for ~~"~ ~.~ , 2011, at ?/~~b d ~./p.m. prevailing time
in Courtroom No. 3 , on the 4th floor of the Cumberland County Courthouse,
Orphan's Court Division, Carlisle, Pa. 17013.
At least twenty (20) days written notice of the hearing on appointment of a
Permanent Plenary Guardian of the person and estate shall be given to Margaret H.
Fick, the alleged incapacitated person, by serving her personally with the Citation and
this Order of Court and a copy of the foregoing Petition together with an explanation of
the content and terms of the Petition.

Additionally, twenty (20) days written notice of the Petition and hearing on
appointment of a permanent guardian shall also be given to the following persons in
this matter: Margaret Bratek and Gary Fick.
