Walsh, a citizen of Pennsylvania, residing at 129 Lucknow Road, Harrisburg, Dauphin County Pennsylvania, 17110. The Respondent is Elena Walsh, a citizen of Pennsylvania, residing at 515 South 18th Street, Camp Hill, Cumberland County, Pennsylvania, 17011. The mother of the child is Elena Walsh, currently residing at 515 South 18th Street, Camp Hill, Cumberland County, Pennsylvania, 17011. She is not married. The father of the child is Michael J. Walsh, currently residing at 129 Lucknow Road, Harrisburg, Dauphin county, Pennsylvania, 17110. He is not married. The relationship of Petitioner to the child is that of father. The Petitioner currently resides by himself. The relationship of Respondent to the child is that of mother. The Respondent currently resides with child and Doug McCurdy. Respondent has not participated as a party or witness, or in another capacity, in other litigation concerning the custody of the child in this or another court. Petitioner has no information of a custody proceeding concerning the child pending in a court of this Commonwealth. Petitioner does not know of a person not a party to the proceedings who has physical custody of the child or claims to have custody or visitation rights with respect to the child. The best interest and permanent welfare of the child will be served by granting the relief requested because he offers the child a more stable and secure environment. Each parent whose parental rights to the child have not been terminated and the person who has physical custody of the child have been named as parties to this action. The Father, Michael J. Walsh, and the Mother, Elena Walsh, shall have shared legal custody of Thomas Michael Walsh, born March 8, 1990. The parents shall consult with each other in making all major non-emergency decisions affecting the Child's general well-being including, but not limited to, all decisions regarding his health, education and religion. Pursuant to the terms of this paragraph each parent shall be entitled to all records and information pertaining to the Child including, but not limited to, school and medical records and information. In the event the parties are unable to reach an agreement on a major decision affecting the Child, the Mother shall have the ultimate decision-making responsibility. 3. The Mother shall have primary physical custody of the Child. 4. The Father shall have partial physical custody of the Child for one weekend each month, upon providing at least 14 days advance notice to the Mother of his selection of weekends. The weekend period of custody shall run from Friday at 4:00 p.m. through Sunday at 1:00 p.m. The Father's first weekend period of custody shall begin on March 1, 2002. In addition, the Father shall have custody of the Child every Wednesday from 4:00 p.m. until 7:00 p.m., beginning February 20, 2002. The Father may have custody of the Child at any additional times as agreed between the parties. 5. The parties shall have custody of the Child on holidays as arranged by agreement. 6. This Order is entered pursuant to an agreement of the parties at a Custody Conciliation Conference. The parties may modify the provisions of this Order by mutual consent.
