Smith, who files this Petition for Custody and in support hereon submit the following: 1. Petitioner, Dondricka L. Elvoid, is the natural mother of the children involved who currently resides at 914 Ricketts Rd. Apt $-D Mechanicsburg, Cumberland County, Pennsylvania. 2. Respondent, Donny L. Elvoid is the natural father of the children who currently resides at 433 Brook Circle, Mechanicsburg, Cumberland County, Pennsylvania 17050. 3.. The Parties are the natural mother and father of the minor children, Dondre L. Elvoid born April 7, 1993 and Donte L. Elvoid born June 19, 1994. 5. From the time the children were until the present the minor children lived with the following persons: April 13, 2001 to the present with mother in Mechanicsburg, Pennsylvania October, 1999-April 13, 2001, with both parents, in Mechanicsburg, DONDRICKA L. CUSTODY TEMPORARY ORn_;R OF COURT AND NOW, this l~lk day of May, 2001, upon consideration of the attached Custody Conciliation Summary Report, it is hereby ordered and directed as follows: 1. ~. The parties, Dondricka L. Elvoid and Donny L. Elvoid, shall have shared legal custody of the minor Children, Dondre L. EIvoid, born April 7, 1993, and Donte L. Elvoid, born June 19, 1994. Each parent shall have an equal right, to be exercised jointly with the other parent, to make all major non-emergency decisions affecting the Children's general well-being including, but not limited to, all decisions regarding their health, education and religion. Pursuant to the terms of Pa. C. S. § 5309, each parent shall be entitled to all records and information pertaining to the Children including, but not limited to, medical, dental, religious or school records, the residence address of the Child and of the other parent. To the extent one parent has possession of any such records or information, that parent shall be required to share the same, or copies thereof, with the other parent within such reasonable time as to make the records and information of reasonable use to the other parent. 2. ~. Effective May 15, 2001, through May 15, 2002, or Mother's return from military duty in Korea, whichever shall be first, Father shall have primary physical custody of the minor Children. Mother shall have rights of partial custody of the Children during school breaks to be exercised in Korea if she can arrange for them to be accompanied from the United States to Korea on an airplane. 3. Father shall make the Children available for reasonable regular telephone contact with their Mother. 4. Mother shall resume as primary custodial parent upon her return from Korea on or about May 15, 2002, so long as she is available to be the primary custodial parent. In the event that she is not available to previde care to the Children for a period of seven days or more, prior to engaging the services of a third party caregiver, Mother shall contact Father to inquire whether he is able and willing to provide for their care. Upon Mother's return Father shall have partial physical custody. In the event that the parties cannot agree to a schedule for Father's partial physical custody, upon proper petition to the Court, the parties may return to the Custody Conciliator for an additional Conference.
