Petitioner is the natural mother of Nicholas Stephen Desantis, bom January 2, 1996 in Cumberland County, pennsylvania. 3. Petitioner's maiden name was Jennifer C. Brooks. 4. Petitioner has been a resident of Cumberland County, Pennsylvania for at least six (6) months previous to the filing of this Petition. 5. Petitioner desires to change the name of her son from Nicholas Stephen Desantis to Nicholas Stephen Berkheimer. 6. The said Dominic Francis DeSantis subsequently challenged paternity. A copyofthe paternity test from Baltimore Ri-I Typing Laboratory excluding Dominic Francis DeSantis as the biological father is attached hereto as Exhibit "B." 9. The natural father of Nicholas Stephen Desantis is Michael P. White. 10. Michael P. White is an adult individual with a mailing address of 6809 Belair Road, Baltimore, Maryland 21206. 11. Michael P. White consents to the proposed name change of Nicholas Stephen Desantis. Attached hereto as Exhibit "C" is a Consent signed by Michael P. White. 12. Nicholas Stephen Dasantis does not have a Social Security Number. WHEREFORE, Petitioner, Jennifer C. Berkheimer, respectfully requests this Honorable Court to issue an Order changing her son's name from Nicholas Stephen Desantis to Nicholas Stephen Berkheimer.
