legally change Minor's name and avers as follows:
1. Petitioner is Erin Asbury, as Parent and Natural Guardian, of Anthony Vincent
Asbury Cucuzella, Minor, 914 Sheffield Avenue, Mechanicsburg, Cumberland
County, PA 17055.
2. Petitioner, Erin Asbury, is the biological mother of Anthony Vincent Asbury
Cucuzella whose date of birth is April 8, 2000, and whose Social Security
Number of 163-80-2032.
3. Thc minor child has resided at the following locations since his birth:
914 Sheffield Avenue, Mechanicsburg, Cumberland County, PA 17055,
(a) from April 8, 2000, until August 24, 2001; and
(b) 1015 Philadelphia Avenue; Chambersburg, PA 17201 from August 24,
2001 to present.
4. The minor's natural father, Alfie Cucuzella (also known as Alfonso Cucuzella),
has no contact with the minor child and Petitioner wishes the child's name to
reflect her last name for consistency as she is the sole custodial parent.

6. The exact location of Alfie Cucuzella (also known as Alfonso Cucuzella) is
unknown. Notice of this Petition shall be provided to his mother's address, his
last known residence, 1159 Resevoir Road, New Holland, PA.
7. Petitioner has properly advertised the Petition and Notice of Hearing information
as required by law.
