Rockey, who respectfully requests this Court to allow him to change his name to Tom Eugene Rockey, and in support thereof avers as follows: I. Petitioner is Thomas J. Kuhn, III, who resides at 1025 Goodyear Road, Gardners, Cumberland County, Pennsylvania, with his mother who is his guardian, Jennie L. Rockey. 2. Jennie L. Rockey was appointed guardian of Thomas J. Kuhn, III, on March 1, 2004, by the Court of Common Pleas of Cumberland County, Pennsylvania. A certified copy of the Order is attached as Exhibit "A". 3. Petitioner has become estranged from his natural father and no longer wishes to have his name. 4. There are no judgments or decrees of record against the Petitioner. 5. Attached to this Petition is a fingerprint card containing the fingerprints of the Petitioner. The Petitioner's fingerprints were placed on the fingerprint card on July ':l.6 _,2004. Said fingerprints are attached for the purpose offorwarding the same to the Pennsylvania State Police as required by 54 Pa. C.S. ~702(b). WHEREFORE, Petitioner respectfully requests this Honorable Court to enter an Order granting the Petitioner's request to change his name from Thomas J. Kuhn, III, to Tom Eugene Rockey.
