During the past five years the Petition~- has resided st the following address: 1711 Charles Street From July 18, 1995 New Cumhedaad, pA 17070 to present 4. The Petitione~ msrried David Asa Entrekin on Februm7 17, 1976 and the couple was divorcad on May 3, 1988. 5. The Petitioner married ~'ames Franklln Kessle~ lli on july 4, 1988 and the couple was divorced un Dec, ember 26, 2000. 6. The Petitioner has no children. 7. The Petiti°ner desires to change the speRin~ of her first name to Nyna to avoid continued mis-pronunciation. The Petitioner desires to drop her freddie name of Lee and so that her name 9. There are no liens against N'ma Lee Loftin.
