Petition to Appeal License Sus ension
I moved to a different residence over a year ago and failed to have my address changed on my license. It'
just did not occur to me to change it and I would have changed it when I renewed my license this October.
Recently I was pulled over for having a headlight out. I had the replacement bulb with me so the officer
let me go with a warning. He pointed out that I needed to change my address and copied down my
current one. I called PENNDOT immediately to have my address changed. Soon after that, I received a
citation in the mail for driving on a suspended license. This was a complete surprise to me! Further calls
and investigation revealed that I was to have received a letter in August telling me I needed to take a
points exam due to a speeding ticket in May 2010. I never received that letter, due to my mistake of not
having changed my address on my license. Apparently my license was then suspended in November, and
again I was completely unaware. When I got the citation, I immediately went to the Magistrate's office to
talk to them and pay my fine. I explained my situation of being in this predicament out of ignorance.
They said I needed to pay the fine regardless, so I did. PENNDOT told me I needed to take a points exam
to have my license restored. I got the book and studied and passed that test this morning, March 21,
2011. It was then that I found out, very much to my surprise, that my license could be suspended for a
year, and I would need to make an appeal. I came to the Carlisle County Courthouse to find out what I
needed to do.
I humbly ask for the mercy of the court. I sincerely apologize for my negligence in not having had my
address changed. I paid $285.50 in fines for driving on a suspended license, even though I did so in
complete ignorance. I have not driven since I found out my license was suspended and would not think of
it! I have learned my lesson thoroughly about paying better attention to detail. I have one more week at
my current job at Keystone Human Services, and on March 28h, I will start a new job at CPARC as a
program advisor. A valid PA license is required for this job. I find myself in a very crucial situation of
needing to have my license restored. Please accept my request for an appeal. Thank you.

privilege has been restored. Only after that may you drive.
The date when you are eligible to have your driving privilege restored
has not been determined. To determine this ELIGIBILITY DATE, you
must resolve any issues listed in this letter as DRIVER'S LICENSE
RETURN, INDEFw!],.TE SUSPENSION, INDEFINITE CANCEL, INDEFINITE RECALL
and/or PRISON r•3LEASE REQUIREMENT. '
Please read the following information carefully and be sure to
complete all requirements to ha-•e your driving privilege restored.
Unless another address is indicated, return any documents and/or
fees to the MAILING ADDRESS listed at the end of this letter.
RESTORATION FEE
-You must pay a $25.00 restoration fee to PENNDOT. Write your
driver's license number (listed above) on the check or money order
to ensure proper credit. Your check or money order should be made
payable to PENNDOT. Please note: Paying the restoration fee DOES NOT
satisfy the requirement to acknowledge your suspension/revocation.
If you have not acknowledged your suspension/revocation, please
follow the instructions listed under "Driver's License Return".

PROOF OF INSURANCE
-Within 30 days of your ELIGIBILITY DATE, provide a copy of one of
the following to PENNDOT to show that all motor vehicles currently
registered in Pennsylvania in your name are insured:
*Insurance ID card
*Declaration page of your insurance policy
*Insurance Binder
*An application of insurance to the PA Auto Insurance Plan
If you do not own a motor vehicle currently registered-in Pennsylvania,
send a signed statement of this fact to PENNDOT which reads "I do
not own any motor vehicles currently registered in Pennsylvania".
Please include your name, address, driver's license number and date
of birth on the statement.

-You are required to return any and all current PENNSYLVANIA driver's
license(s), learner's permit(s) and/or camera card(s) to PENNDOT.
If these are not in your possession, are expired, or you have never
been issued a PENNSYLVANIA driver's license, complete and return an
Acknowledgment Form (DL-16LC) to PENNDOT.
If you are an OUT OF STATE licensed driver or an OUT OF STATE resident
who has never been issued a driver's license, complete and return an
Acknowledgment Form (DL-16LC) to PENNDOT.
An Acknowledgment Form (DL-16LC) may be obtained at any PENNDOT
Driver's License Center. You may also write or call the address or
phone number at the end of this letter to obtain this form.
TERM SUSPENSION/REVOCATION
-You have a 1 YEAR(S) suspension/revocation that began
(or will begin) on 03/14/11. Refer to the DRIVER'S LICENSE RETURN
section in this letter for information on getting credit started
for serving this suspension/revocation. The suspension/revocation
is a result of 1543, DRIVING WHILE SUSPENDED OR REVOKED
