AND NOW, comes the Executor of the Estate of Gayle Harlin Kluz, by and through his
attorney, Melissa P. Tanguay, Esquire of ABOM & KUTULAKIS, L.L.P., to amend the Letters
Testamentary as follows:
1. On February 14, 2011, Letters Testamentary were granted to Christopher J. Speece, Executor
of the Estate of Gayle Harlin Kluz.
2. The Executor then became aware that the Social Security Number for Gayle Harlin I{luz was
incorrect on the Petition for Probate and Grant of Letters and Estate Information Sheet
previously flied with 'This Honorable Court
3. The correct Social Security Number for Gayle Harlin Kluz is 513-52-2809.
4. The Executor is requesting that This Honorable Court amend all previously issued documents
to reflect the correct information for Gayle Hazlin Kluz.
5. An amended Death Certificate for Gayle Harlin Kluz is attached.
6. An amended Estate Information Sheet is being filed simultaneously with this petition.
WHEREFORE, the Executor of the Estate of Gayle Harlin Kluz respectfully requests that This
Honorable Court amend the Certificate°of Grant of Letters and Short Certificate to reflect the convect
Social Security Number for Gale Hazlin Kluz.
