AND NOW, this 22nd day of March, 2011, upon consideration of the Petition to Amend Petition for
Probate and Grant of Letters and an amended death certificate for the decedent, Gayle Harlin Kluz, filed by
Christopher Speece and Melissa P. Tanguay, Esquire, IT IS DECREED that the probate record shall be
amended to reflect the correct Social Security Number as 513-52-2809. IT IS FURTHER DECREED that after
the correction is made, Letters Testamentary shall be issued reflecting the correct Social Security Number and the
Register of Wills shall notify the Pennsylvania Department of Revenue, Inheritance Tax Division of the change to
the estate record.
