An alleged incapacitated person ~ c~ -~ ;~' ` -_~' N0. 21-11- 0253 t 7i ~+~ r` na ~~ ' ~ «3a _„ .. ~~ r _ _~ FINAL DECREE ~ ~ ~ ~= 1~- ~ -Y~ AND NOW, this ~~ day of March, 2011, upon consideration of the Petition for Appointment of a Permanent Plenary Guardian of the Person and Estate of Heinz Juhl pursuant to 20 P.S. Section 5513, and following hearings at which evidence was presented by the Petitioner in support of the Petition, and, based upon the foregoing, it is hereby ORDERED and DECREED: 1. That Heinz Juhl is an incapacitated person in that he suffers from a mental impairment that prevents him from receiving and evaluating information effectively and prevents him from formulating and communicating decisions to such a significant extent that he is unable to manage his financial affairs or meet essential requirements for his physical health and safety on a daily basis, and is adjudicated as such. 2. That the Court appoints Cumberland County Aging & Community Services as Permanent Plenary Guardians of the Person and Estate of Heinz Juhl. No bond shall be required. 3. That the Guardians shall file on a timely basis with the Court all reports and accounts required by statute. 4. That the Guardians be granted payment of a Guardian fee pursuant to the fee schedule attached to the Petition, marked as Exhibit "B", and, at such time that Heinz Juhl no longer has sufficient assets to pay for the services, the Guardians shall be entitled i J to a maximum of 10%~of Heinz Juhl `s gross monthly income or $100.00 per month, whichever is less, purs~ant'to medical assistance regulations.
