PERSONALLY came and appeared before me, the undersigned Notary, the within
named Michael H. Warner, who is a resident of Lancaster County, State of
Pennsylvania, and makes this his/her statement and General Affidavit upon oath
and affirmation of belief and personal knowledge that the folic+wing matters, facts
and things set forth are true and correct to the best of his/her knowledge:
1. The last will and testament of my father, Kermit L. Warner., Sr. of West
Fairview, Pennsylvania contains two typographical errors in the suffix of his name
which incorrectly identify him as Kermit L. Warner, Jr. on Page 1 of the document.
2. The remainder of the last will and testament of Kermit L. Warner, Sr. correctly
identifies him as Kermit L. Warner, Sr.
3. The signature line of the last will and testament of my father, Kermit L. Warner,
Sr. correctly identifies him as Kermit L. Warner Sr.
4. My father, Kermit L. Warner, Sr., did hire attorney, Robert E. Myers, 100 York
Road, New Cumberland, Pennsylvania, 17070 to prepare his last will and
testament in the month of October in the year 2003.
5. This last will and testament of Kermit L. Warner, Sr. was signed and notarized
on October 16, 2003.
6. This last will and testament was witnessed by Attorney, Robert E. Myers of
New Cumberland, Pennsylvania and Joanne Shindel of Etters, Pennsylvania.
7. Robert E. Meyers is currently nonverbal due to serious health problems. He is
no longer practicing law. Joanne Shindel is deceased.
8. My father, Kermit L. Warner, Sr., shared his last will and testament with me in
October, 2003 to inform me of his decision to appoint me, Michael H. Warner, as
Executor of his last will and testament. I am familiar with my father's signature
and recognize it as the signature which appears on pages 4 and 5 of his last will
and testament.

9. I, Michael H. Warner, declare that the last will and testament. prepared by
Robert E. Meyers of New Cumberland on the 16th Day of October, 2003 is that of
my late father, Kermit L. Warner, Sr.
