ORDERED that the settlement and proposed distribution of the settlement proceeds set
forth in the attached Petition are approved, and the proceeds from such settlement shall
be paid as follows:
1. The sum of $50,000 shall be paid by Nationwide Insurance to the
ESTATE OF GRACE A. BRANTHAFER, Deceased, in the survival action claim
brought as a result of the death of Grace A. Branthafer.
2. From the settlement amount, the sum of $16,500 shall be paid to Salzmann
Hughes, P.C. as attorney fees.
