CUSTODY STIPULATION
It is hereby stipulated that:
1. Melissa J. Smith, natural mother of herein-named minor, is an adult individual who currently resides
at 391 Easy Rd., Carlisle, Cumberland County, PA 17055.
2. Clair E. Barrick Jr., natural father of herein-named minor, is an adult individual who currently resides
at 128 Darr Ave., Carlisle, Cumberland County, PA 17013.
3. Amber L. Barrick, minor and natural child of Clair E. Barrick Jr. and Melissa J. Smith currently resides
at 391 Easy Rd., Carlisle, PA 17055.
4. Clair E. Barrick Jr. and Melissa J. Smith have shared legal and physical custody of Amber L. Barrick.
Clair E. Barrick Jr. wishes to give custody of Amber L. Barrick to Melissa J. Smith. Melissa J. Smith
will have full, legal and physical custody of Amber L. Barrick, including the right and responsibility to
make decisions regarding Amber L. Barrick's education, health, financial and any other decision that
may be required to properly care for said minor.
6. Melissa J. Smith wishes to accept full, legal and physical custody of Amber L Bardck.
7. Clair E. Barrick Jr. will pay $320.56 a month in child support to Melissa J. Smith.
