That the Petitioners are of full legal age and are bona fide residents of the County of Cumberland , Commonwealth of Pennsylvania, whose residence address is 108 Fieldstone Drive, Carlisle, PA 17015 . Petitioners have been bona fide residents of Cumberland County, Commonwealth of Pennsylvania for 14 years immediately prior to filing for this petition. Petitioners were born on the 12th day of April 1946 , in the County of Los Angeles , State of California ; and on the 28th day of December , 1955 , in the County of Cumberland , State of North Carolina . Petitioners have lived in the following places for the past five years preceding the filing of said petition: Dates (from/to) Address 08/00 / Now 108 Fieldstone Drive, Carlisle, PA 17015 2. Petitioners' present names are Robert Brian Jones & Lucia Ann Jones . These are the current names of the Petitioners on their Social Security Cards and all other records. This Petition does not request that the court order a change in the surname of the minor children to the same surname ordered for the Petitioners. 7. Petitioners: Q Have never been convicted of a felony. ? Were convicted of a felony on (date) of the charge of by the Court in County, State of and ? at least two calendar years have elapsed from the date of completion of a person's sentence and the Petitioner is not subject to probation or parole jurisdiction of any court, county probation agency of the Pennsylvania Board of Probation and Parole; or ? the Petitioner was pardoned on (date). 8. That the Petitioners pray for the change of name for the following reasons (describe in detail): We wish to restore our old family name because Robert's father was adopted by his mother's second husband. 9.
