AND NOW, this D day of J`t ~ , 2011, upon review and
consideration of the Petition of Mylesetta R. Hoffman to have her Daughter, Marilyn J. Webber,
Declared an Incapacitated Person and for her Appointment as Guardian of the Person and the
Estate of Marilyn J. Webber, and pursuant to due notice of a hearing and evidence received, the
following is hereby finally DECREED:
1. Marilyn J. Webber is hereby decreed a totally incapacitated person due
to a mental condition pursuant to P.A. C.S.A. §§ 5511 and 5512.1, and other
applicable statutory provisions.
2. Mylesetta R. Hoffman, Petitioner, mother of Marilyn J. Webber, is
appointed Plenary Guardian of the Person of Marilyn J. Webber, and Plenary
Guardian of the Estate of Marilyn J. Webber, for an unlimited duration of time. In
the event Mylesetta R. Hoffman for any reason no longer can serve or chooses to
no longer serve as Guardian of the Person andlor Guardian of the Estate of
Marilyn J. Webber, then in that event Donald Klaiber shall serve as Plenary
Guardian of the Person of Marilyn J. Webber and/or Plenary Guardian of the
Estate of Marilyn J. Webber.
3. In the event Marilyn J. Webber's total incapacitation shall become
limited in scope, or her incapacitation entirely ceases to exist, and such is
satisfactorily demonstrated to this Court either upon motion by Marilyn J. Webber,

any Attorney or representative on her behalf, or the Guardian appointed herein,
then in that event this Court shall order limits upon or cessation of the plenary
powers of the Guardian of the Person and the Guardian of the Estate as it deemed
appropriate_in conformance..with. the_ evidence presented
4. Mylesetta R. Hoffman and Donald Klaiber are respectfully appointed as
Guardian Ad Litem and back-up Guardian Ad Litem for the purpose of initiating a
claim for Social Security Disability Benefits for the benefit of Marilyn J. Webber.
5. Marilyn J. Webber is hereby advised of and assured that she has the
right to appeal this Final Decree and the right to petition to modify or to terminate
the Guardianships decreed herein.
