ACCEPTANCE OF SERVICE AND DISCLAIMER
I, FRANK JAMES CEKOVIC, hereby aver as follows:
1. I am FRANK JAMES CEKOVIC, currently residing at 6040
Wertzville Road, Enola, Pennsylvania.
2. I am a son of ELI CEKOVIC and ELLEN MARY MESSINGER
CEKOVIC.
3. I hereby accept service of the Notice of Hearing in the matter of
a Petition for Settlement of Survival Action pertaining to the above Estate. The
hearing is scheduled for April 29, 2011, at 2:30 p.m. in Courtroom Number 1,
Cumberland County Courthouse, Carlisle, Pennylvania.
4. I hereby renounce any benefit from a survival action or a
wrongful death action. It is my desire that all proceeds from this action should
be payable to my mother, ELLEN MARY MESSINGER CEKOVIC.
5. I am self-supporting, and I derived no financial benefit from my
father during his lifetime. I have suffered no financial loss as a resu~t of my ;.,,
