TO THE APPROPRIATE AUTHORITY AT GALLAGHER BASSETT
In the Civil District Court for the State of Louisiana, Parish of Orleans, there is
pending a case entitled: "Vincent Henderson v. Michael Denby and Chartis Insurance
Company" and it appears to the Court that the just determination of the issues therein
presented requires that a subpoena duces tecum be issued and served upon the records
custodian at Gallagher Bassett at 8 Flowers, Mechanicsburg, PA 17050.
It is therefore requested that you assist this Court in serving the interests of justice by
causing the records custodian to produce its complete records of Plaintiff, Vincent
Henderson, on or before its records only deposition scheduled for the 19th day of May, 2011
at 10:30 a.m. at the offices of Dudley Debosier, 1124 St. Charles Avenue, New Orleans, La
70130 as set forth in the attached Subpoena Duces Tecum and Notice of Records Only
Deposition. This Court stands ready and willing to extend the same courtesies to the State of
Pennsylvania in a similar case when required.
