PETITION FOR CHANGE OF NAME
TO THE HONORABLE JUDGES OF SAID COURT:
NOW comes the petitioner, ROBERT WALTER LISZEWSKI, JR., by his
attorney, Harold S. Irwin, III, Esquire, and presents this petition for change of name,
representing as follows:
1. Petitioner is ROBERT WALTER LISZEWSKI, JR., an adult individual
residing at 9 Front Street, Boiling Springs, Cumberland County, Pennsylvania 17007.
2. Petitioner was bom on February 17, 1972 to Robert Walter Liszewski and
Patricia JoAnn Wetmore, and is now 32 years old. A copy of petitioner's birth certificate
is incorporated herein by reference and attached hereto as Exhibit "A".
3. Petitioner's parents divorced sometime in 1973 - 1974 and petitioner
resided thereafter with his mother. In about 1976, his mother married Kenneth Moore.
Thereafter, from the time that petitioner was about four years old, petitioner's mother
and stepfather began calling petitioner by the name, Thomas Jonathan Moore.
4. For the past 28 years, petitioner has been known to all of his family,
fdends and the public as Thomas Jonathan Moore. For example, his driver's license,
social secudty account and college degree all list petitioner as Thomas Jonathan
Moore. Petitioner has never held himself out as Robert Walter Liszewski, Jr. since the
age of four years.
5. Petitioner now desires to have his name formally changed to Thomas
Jonathan Moore, the name by which he has been known nearly his entire life.
6. There are no judgments or decrees of like character of record against
Robert Walter Liszewski, Jr.
WHEREFORE, petitioner requests your Honorable Court, pursuant to Act of
December 16, 1982, P.L. 1309, No. 295, Section 2, 54 Pa. Cons. Stat. Section 701, et
seq., to do the following:
August 6, 2004
Liszewski, Jr., to Thomas Jonathan Moore.
Fix a hearing on this petition;
Direct that notice be given of this petition; and
Enter a decree changing the name of petitioner from Robert Walter
VERIFICATION
The foregoing petition is true and correct to the best of my knowledge,
information and belief. I understand that false statements made herein are subject to
the penalties of 18 Pa.C.S.A. Section 4094, relating to unsworn falsification to
authorities.
