Boswell, Esquire, and BOSWELL, TINTNER & PICCOLA, hereby requests this Honorable Court order a change of name pursuant to 54 Pa.C.S.A. § 701, et seq., and in support thereof represents, as follows: 1. Petitioner, Alyssa Lynn Loney (herein referred to as "Petitioner"), was born on November 22, 1969, and is over the age of twenty-one (21) years. 2. Petitioner is a resident of the Borough Carlisle, Cumberland County, Pennsylvania, and has resided at the following locations during the previous five (5) years: a. Petitioner currently resides at 145 North Hanover Street, Carlisle, Cumberland County, PA 17013, and has done so since July, 2002; b. Prior thereto, Petitioner's permanent address had been 1215 Hillside Drive, Carlisle, Pennsylvania 17013. fy2, o? O L q"Vy P-ro 3. Petitioner is a graduate of West Virginia University, Morgantown, West Virginia, having been awarded the degree of Bachelor of Science in Textiles and Clothing, as well as the University of York in the United Kingdom, having been awarded the degree of Master of Arts in Medieval Archeology. 4. Petitioner is currently a certified appraiser of antiques, heirlooms, cultural property, and various personal property items. Petitioner maintains an office at 145 North Hanover Street, Carlisle, Pennsylvania. 5. Over the course of her lifetime, Petitioner has been known as Alyssa Lynn Loney or as Alyssa L. Loney. 6. Petitioner desires that her formal and legal name be changed to "Alyssa Lillian Wardecker Loney." Petitioner desires that her legal identity be indicated as "Alyssa Lillian Wardecker Loney" in all future licenses, certifications, and other documentation associated with her profession, as well as all other personal identifying material such as her driver's license and her passport. Documents containing her present legal name are included in this Petition, including exact copies of her Pennsylvania driver's license, which is Exhibit "A," her United States Passport, which is Exhibit "B," and her Birth Certificate, which is Exhibit "C." 7. The Petitioner requests that her formal and legal name be changed to "Alyssa Lillian Wardecker Loney" to reflect her lineage, as a descendent of her grandmother, Anna Lillian Davis. The said Anna Lillian Davis, upon her marriage, became known as Ann Lynn Wardecker, because she informally assumed this name, changing slightly her given first and second names. Petitioner's grandmother, just prior to her death, disclosed to her family that the name given to her upon her birth was actually Anna Lillian, disclosing that she had favored, and, therefore, assumed as her name Ann Lynn rather than Anna Lillian, her given name. 8. Petitioner has no judgments or decrees of like character of record. 9. Petitioner has had no prior criminal convictions for either felony or misdemeanor offenses, and she faces no pending charges of a like nature. Petitioner has included her fingerprints as part of the instant petition so that the Court can forward same to the Pennsylvania State Police for an appropriate investigation verify this averment, which fingerprint record is attached as Exhibit "D." 10. Petitioner has no illegitimate purpose for her seeking the requested change of her proper and legal name. 11. Petitioner reflects that this Honorable Court enter an order legally changing her name to "Alyssa Lillian Wardecker Loney" to reflect properly her descent from her grandmother. WHEREFORE, Petitioner Alyssa Lynn Loney respectfully requests that this Honorable Court enter an Order of Court legally changing her name from "Alyssa Lynn Loney" to "Alyssa Lillian Wardecker Loney."
